New Angels Child Care Pre-School

In accordance with the norms of a licensed child care provider, the following documents must be completed prior to offering child care services. These documents are included in the admission package:

1. Parent/Provider Contract/Enrollment Application

2. Health Inventory and Addendum

3. Copy of Child’s Current Immunization Record Signed by a Doctor.

4. Copy of Lead Test Signed by a Doctor

5. Emergency Information/Consent for Medical Treatment
